Accessing the service

Referrals
We accept referrals from a wide range of services including self-referralmember of staff
• Community drug and alcohol services;
• GPs and primary care teams;
• Drug intervention services;
• Probation, prison and criminal justice teams.

Our Referral Service offers the following:
• We will take initial details and complete a referral from with clients and professionals;
• We will provide information on support and housing options available to clients and referrals;
• We will arrange and carry out needs and risk assessments at the service of choice or at other venues if this is not possible;
• We will provide pre-admission support to clients including information on contacts for health, housing and childcare issues;
• We will liaise with other agencies involved in the client's support plan to support the care pathway.
We will visit other agencies to promote and describe our services.
